=== Stereocilia === The stereocilia of the epididymis are long cytoplasmic projections that have an actin filament backbone. These filaments have been visualized at high resolution using fluorescent phalloidin that binds to actin filaments. The stereocilia in the epididymis are non-motile. These membrane extensions increase the surface area of the cell, allowing for greater absorption and secretion. It has been shown that the epithelial sodium channel ENaC that allows the flow of Na+ ions into the cell is localized on stereocilia. Because sperm are initially non-motile as they leave the seminiferous tubules, large volumes of fluid are secreted to propel them to the epididymis. Since 26 May 2025, the United States- and Israeli-backed Gaza Humanitarian Foundation has facilitated limited and largely ineffective aid distribution. In June 2025, UNICEF-supported clinics in Gaza reported a sharp rise in child malnutrition: 16,736 children were treated between January and May 2025, including 5,119 in May alone. This was nearly 50% more than in April and over double the February figure. Of these, 636 had severe acute malnutrition, the deadliest form, which has risen by about 146% since February. UNICEF warns that without large-scale aid deliveries, acute malnutrition could reach its highest level since the conflict began, despite wasting being virtually absent 20 months earlier.
== Diagnosis == The diagnosis is based on involvement of less than 10% of the skin. It is known as TEN when more than 30% of the skin is involved and an intermediate form with 10 to 30% involvement. A positive Nikolsky's sign is helpful in the diagnosis of SJS and TEN. A skin biopsy is helpful, but not required, to establish a diagnosis of SJS and TEN.
== Risks == The risks are similar to other intraocular procedures, but additionally include graft rejection (lifelong), detachment or displacement of lamellar transplants and primary graft failure. Use of immunosuppressants including cyclosporine A, tacrolimus, mycophenolate mofetil, sirolimus, and leflunomide to prevent graft rejection is increasing but there is insufficient evidence to ascertain which immunosuppressant is better. In a Cochrane review which included low to moderate quality evidence, adverse effects were found to be common with systemic mycophenolate mofetil, but less common with topical cyclosporine A or tacrolimus. There is also a risk of infection. Since the cornea has no blood vessels (it takes its nutrients from the aqueous humor) it heals much more slowly than a cut on the skin. While the wound is healing, it is possible that it might become infected by various microorganisms. This risk is minimized by antibiotic prophylaxis (using antibiotic eyedrops, even when no infection exists). There is a risk of cornea rejection, which occurs in about 10% of cases. Graft failure can occur at any time after the cornea has been transplanted, even years or decades later. The causes can vary, though it is usually due to new injury or illness. Treatment can be either medical or surgical, depending on the individual case. An early, technical cause of failure may be an excessively tight stitch cheesewiring through the sclera. Infectious disease transmission through corneal transplantation is exceedingly rare.

== Location and characteristics == The location of prolactin cells within the pituitary gland is regulated largely by the hypothalamus. The pituitary gland is divided into posterior and anterior regions. Within the anterior pituitary gland are the prolactin cells, where they secrete the hormone prolactin. Prolactin cells vary in number, size, and appearance depending on female reproductive status. Prolactin cells specifically increase in response to the physiological state of pregnancy, in particular, the need for the development of breast tissues and milk production. During pregnancy, prolactin cells will undergo hypertrophy (enlarging to support increased prolactin production) as well as hyperplasia (an increase in cell number). The pituitary gland increases in size due to the amount of prolactin cells. The secretory granules of prolactin cells fluctuate from sparsely granulated (during periods of low prolactin production) to densely granulated (during periods of high prolactin production). Prolactin cells contain a large amount of rough endoplasmic reticulum, where prolactin synthesis occurs. The trans-Golgi layer is responsible for storing the prolactin hormone into secretory granules, which are dissolved upon secretion out of the cell. Lysosomal enzymes are involved in the degradation of the secretory granules. The prolactin hormone is a single polypeptide chain protein composed of 199 amino acids in humans. It consists of “three intramolecular disulfide bonds located between six cysteine residues (Cys4-Cys11, Cys58-Cys174, and Cys191-Cys199”.
=== Legacy === Hahn is considered the father of radiochemistry and nuclear chemistry. He is chiefly remembered for the discovery of nuclear fission, the basis of nuclear power and nuclear weapons. Glenn Seaborg wrote that "it has been given to very few men to make contributions to science and to humanity of the magnitude of those made by Otto Hahn". His award of the 1944 Nobel Prize for Chemistry was in recognition for this discovery. However later commentators have argued that Lise Meitner's exclusion reflected sexism and antisemitism within the Nobel Committee. Conflict between chemists and physicists and the theorists and experimentalists also played a role. Hahn's efforts to rehabilitate the image of Germany after the war have also been viewed as problematic. Hahn has been described as politically passive during the Nazi era, suggesting that while he was not a party member, he tolerated colleagues who were and thus shared moral complicity. == Pathophysiology == Multiple pathophysiological changes have been observed in PAH. This includes an imbalance in apoptosis (programmed cell death) and proliferation of endothelial cells, resulting in intimal thickening as well as proliferation and hyperplasia of the smooth muscle cells constituting the muscular layer of the pulmonary arteries. The smooth muscles in the tunica media also extend more distally than normal, encroaching upon the capillary bed. Infiltration of inflammatory cells, proliferation of fibroblasts and disruptions in collagen architecture result in adventitial thickening and remodeling. All of these changes combine to lead to thickening of the pulmonary arteries and arterioles with an associated increase in pulmonary arterial resistance (increased pulmonary artery pressure). Pathogenic and inappropriate platelet activation coupled with endothelial injury leads to formation of micro-thrombi. And PAH also involve the characteristic plexiform lesions which are growths in the walls of the arterioles consisting of dilated blood vessels which communicate with the bronchial artery and vaso vasorum. As pulmonary hypertension persists and worsens the right ventricle undergoes compensatory changes such as concentric hypertrophy of the heart muscle and changes in the microcirculation. However, with prolonged pulmonary hypertension, with the right ventricle pumping against elevated right heart pressures, the hypertrophy becomes maladaptive with microvascular rarefaction, and fibrosis. These changes eventually culminate in right heart failure.
== Diagnosis == There are some laboratory tests that may aid in the diagnosis of GSD-V. A muscle biopsy will note the absence of myophosphorylase in muscle fibers. In some cases, abnormal glycogen accumulation stained by periodic acid-Schiff can be seen with microscopy. Genetic sequencing of the PYGM gene (which codes for the muscle isoform of glycogen phosphorylase) may be done to determine the presence of gene mutations, determining if McArdle's is present. This type of testing is considerably less invasive than a muscle biopsy. The physician can also perform an ischemic forearm exercise test as described below (see History). Some findings suggest a nonischemic test could be performed with similar results. The nonischemic version of this test would involve not cutting off the blood flow to the exercising arm. Findings consistent with McArdle's disease would include a failure of lactate to rise in venous blood and exaggerated ammonia levels. These findings would indicate a severe muscle glycolytic block. Serum lactate may fail to rise in part because of increased uptake via the monocarboxylate transporter (MCT1), which is upregulated in skeletal muscle in McArdle disease. Lactate may be used as a fuel source once converted to pyruvate. Ammonia levels may rise given ammonia is a by-product of AMP deaminase which follows after the production of AMP by adenylate kinase, an alternative pathway for ATP production.
=== Limitations === The minimization of radiation dose to the subject is an attractive feature of the use of short-lived radionuclides. Besides its established role as a diagnostic technique, PET has an expanding role as a method to assess the response to therapy, in particular, cancer therapy, where the risk to the patient from lack of knowledge about disease progress is much greater than the risk from the test radiation. Since the tracers are radioactive they are generally not used with those who are pregnant. Limitations to the widespread use of PET arise from the high costs of cyclotrons needed to produce the short-lived radionuclides for PET scanning and the need for specially adapted on-site chemical synthesis apparatus to produce the radiopharmaceuticals after radioisotope preparation. Organic radiotracer molecules that will contain a positron-emitting radioisotope cannot be synthesized first and then the radioisotope prepared within them, because bombardment with a cyclotron to prepare the radioisotope destroys any organic carrier for it. Instead, the isotope must be prepared first, then the chemistry to prepare any organic radiotracer (such as FDG) accomplished very quickly, in the short time before the isotope decays. Few hospitals and universities are capable of maintaining such systems, and most clinical PET is supported by third-party suppliers of radiotracers that can supply many sites simultaneously.
